The Three Zones of a Medical Office
Every medical facility can be broken into three cleaning zones with different frequencies, chemistries, and dwell times.
Zone Matrix
| Zone | Examples | Frequency | Chemistry |
|---|---|---|---|
| Patient care | Exam rooms, procedure rooms | Between every patient | EPA List N, 3-10 min dwell |
| Public | Waiting rooms, restrooms, reception | Multiple times daily | Quat-based sanitizer |
| Support / admin | Back offices, break rooms | Nightly / weekly | General-purpose cleaner |
Cleaning vs. Disinfecting vs. Sterilizing
- ✓Cleaning removes soil — always precedes disinfecting.
- ✓Disinfecting kills pathogens on surfaces (EPA List N for SARS-CoV-2).
- ✓Sterilizing eliminates all microbial life — reserved for instruments, not surfaces.
High-Touch Points in Medical Offices
- ✓Door handles, cabinet pulls, drawer knobs
- ✓Exam table paper dispenser, stool wheels
- ✓Blood pressure cuff, stethoscope diaphragm
- ✓Countertops, sink handles, soap dispensers
- ✓Reception glass, sign-in tablets, pens & clipboards
- ✓Restroom fixtures, grab bars, flush levers
OSHA Bloodborne Pathogen Basics
OSHA 29 CFR 1910.1030 requires written exposure control plans, PPE, sharps handling, and post-exposure procedures. Contract cleaning teams must be trained annually and their training documented — ask your vendor for records.
Documentation to Keep on File
- ✓SDS binder for all products in use
- ✓Vendor OSHA training records (annual)
- ✓Certificate of Insurance ($2M general liability minimum)
- ✓Cleaning logs by room and shift
- ✓Quarterly quality-inspection reports
When to Escalate to Terminal Cleaning
Terminal cleaning (full room strip and disinfect) is warranted after known infectious cases (C. diff, TB), major biohazard events, or before major inspections. It typically requires 45-90 minutes per exam room.
Red Flags in Your Current Program
- ✓Cleaner uses same rag/mop across multiple exam rooms
- ✓No color-coded microfiber system
- ✓Dwell time on disinfectant is under 3 minutes
- ✓Products in use are not EPA List N
- ✓No visible cleaning log in restrooms
